How to act in case of:

Death of the insured

To claim a death indemnity, complete the application for an indemnity payment and submit it to us with the death certificate. You can send the documents to [email protected] or deliver them to our Tallinn office at Keemia 4, Tallinn, 10616.

Critical illness

You can notify us about an illness either on E-life, our online customer website, or by sending an e-mail to us at [email protected] or phoning 681 2300. After submitting the claim report, please submit the following documents to our Tallinn office:

1. An Application for Indemnity completed by the applicant.
2. Medical documents regarding the diagnosis of the critical illness and the treatment performed. For more information, please contact us by phone at 681 2300 or by e-mail to [email protected].

 

Accident resulting in injury or disability

In case of an accident, please notify us of what happened as quickly as possible, preferably within 3 working days of the accident.

You can let us know about the accident either on E-life, our online customer website, or by sending an e-mail to us at [email protected] or phoning us at 681 2300.

After submitting the claim report, please submit the following documents to our office:

1. An Application for Indemnity completed by the applicant
2. A doctor’s certificate on the Mandatum Life template (in Estonian) completed by the doctor (if the contract includes a daily allowance)
3. A certificate of incapacity for work (if the contract includes a daily allowance)
4. In case of bone fractures, your X-rays
5. When claiming a disability indemnity, medical documents confirming permanent disability
6. For a work accident, the work accident report
7. For an injury, documents confirming treatment (hospital records)
8. For traumas, insurance indemnity will be paid if you have supplementary accident insurance cover with us.
